Find forms at the bottom of this page for legal name change and for changing gender on your birth certificate. Usually best to change your name legally first, and then update the birth certificate with the new name and new gender designation. This is only for those born in BC. If you were born in another province, you will need to see what provisions they have. Manitoba and Ontario have developed ways to change your gender without surgery, but others have not, or are in the process. Scenario #1: Proof of citizenship under new sex:
If you wish to have a passport in your new given name and gender, you should submit a completed passport application form, your original proof of Canadian citizenship indicating the new gender and the new given name, two pictures, a document to support your identity under the name requested and the required fee. Also, it is essential that you include the valid passport you currently hold with your application for cancellation.
Scenario #2: Proof of citizenship under previous gender:
If you have undergone sex reassignment surgery and your proof of citizenship does not show the new sex, the following documents must be submitted:
A full validity passport may be issued in the new sex.
Scenario #3: Pre-surgery:
A limited validity passport may be issued in an assumed sex to an applicant who provides medical documentation indicating that he or she will be undergoing sex reassignment surgery in the next twelve months. Please be advised that the passport will be valid for two years and will not be extended.
You will also have to complete and sign a Request for a Canadian passport indicating a sex other than the sex shown on my documentary evidence of citizenship, Form PPTC 152, acknowledging awareness that you may encounter difficulties with a passport issued showing your assumed sex.
